Vaccination Clinic for Children Ages 5-11 Years Old

Registration Information for COVID-19 Vaccination Clinic at Mockingbird Elementary

Ralston Public Schools and OneWorld Community Health Centers are partnering to offer a Children’s Vaccination Clinic at Mockingbird Elementary (5100 S. 93rd St.) on Friday, November 19, 2021. The clinic will run from 3:30 to 6:30 pm in the school gymnasium. This clinic will be for children ages 5 to 11 years of age.

As a District, we fully respect that receiving a vaccination is a choice for families. Throughout the pandemic, RPS has continually shared information from our medical community. We will continue to provide information from our local health professionals as a resource for our families.

How to Register & Clinic Information
Appointments are strongly encouraged. You can schedule your child(ren)'s vaccination time online using the linked SignUpGenius form below. Once you select your time, you will be able to reserve it by entering your child’s name and date of birth.

Walk-in appointments will also be available but will be limited. On November 19th, parents will need to accompany their child(ren) to the clinic. Masks will be required by all who attend the clinic. Interpreters will be present to assist families.

Important Document to Bring with You - Parental Consent Form
Below is a link to the OneWorld Community Health Center vaccination consent form. Parents, please fill out the consent form ahead of time and bring it to the vaccination clinic. It will make the registration process faster for all.

If you can't print the consent form, paper copies will be available for you to fill out at the clinic. It is available in both English and Spanish.
