Parents or students who desire to have student fees waived must complete this form. In order to verify eligibility for the fee waiver, an Application for Free and Reduced Price Meals must be completed and submitted (unless already on file with the district). Application for the fee waiver may be made at any time, but must be renewed annually. All activities for which a waiver is sought should be listed on this form.
